COMMUNITY SERVICE GUIDELINES

Please read the 2016-2017 Guidelines.

SUMMER HOURS WERE DUE BY SEPTEMBER 2, 2016. All hours were collected by end of the day on Friday, September 2.  Any summer hours turned in after that date are not recorded.

MAKE SURE FORMS ARE COMPLETE AND LEGIBLE- PLEASE PRINT ALL INFORMATION EXCEPT SIGNATURE OF SUPERVISOR.  
NO NICKNAMES - SUBMIT THE STUDENT'S FIRST AND LAST NAME.

All forms MUST have:

1.   NAME - FIRST & LAST - NO NICKNAMES
2.   STUDENT ID
3.   GRADUATION YEAR CIRCLED
4.   DATE SUBMITTED
5.   DATE(S) OF SERVICE
6.   NUMBER OF HOURS
7.   HOMEROOM TEACHER
8.   NAME OF NON-PROFIT ORGANIZATION
9.   LOCATION OF ORGANIZATION
10. DESCRIPTION OF SERVICE PERFORMED
11. SIGNATURE OF SUPERVISING ADULT (CANNOT BE A PARENT OR GUARDIAN) AND THEIR PHONE NUMBER.

**INCOMPLETE FORMS WILL BE HELD AND HOURS NOT RECORDED UNTIL THEY ARE FILLED OUT. NAMES WILL BE ANNOUNCED TO SEE MRS. SPRENGER IN ROOM 217B SO THAT STUDENTS CAN COMPLETE THEIR FORMS.**

Please don't turn in forms to the office.  It is the student's responsibility to have the forms completed and signed and turn them in to Mrs. Sprenger in Room 217B.
